Frequently Asked Questions - Enhanced Life Insurance Coverage

1. What is my PSAC ID number?

Your PSAC ID number is located on your PSAC Membership Card. If you do not have your card on hand, you may contact our office at 1-800-216-1107 or contact your PSAC Regional Office.

2. Can I apply for the PSAC Enhanced life insurance coverage?

If you are a PSAC member in good standing who is actively at work and under the age of 65, you are eligible to apply for the PSAC Enhanced Life Insurance coverage from $25,000 to a maximum of $250,000.

3. How do I apply for the PSAC life insurance coverage?

Simply complete the PSAC Enhanced Coverage Application Form and return it to our office by mail only to the following address:

4. How much does the coverage cost?

Premiums are based on age, sex, smoking status and the amount of coverage you require. Please refer to the rate table for details.

5. How long will it take to process my application?

Once our office is in receipt of your completed application, the average insurer response time is four to six weeks. The medical underwriting process may take longer if the insurance company requires additional medical information.

6. Can I get life insurance for my spouse under the plan?

Yes. If you are a PSAC member in good standing who is actively at work and under the age of 65 (as well as your spouse), you are eligible to apply for spousal life insurance coverage in the amount of $25,000 to a maximum of $250,000.

7. Can I add my dependent child to the plan?

Yes. If you are a PSAC member in good standing who is actively at work and you or your spouse is insured under the plan.

8. How much life insurance coverage can I get for my child(ren) and how much does it cost?

Each dependent child can be insured for $20,000 for only $1.00 per month.

9. How long can my child(ren) stay insured?

As long as you or your spouse continue to be insured under the plan, your dependent child(ren) can remain insured until the age of 19 or until the age of 25 if s/he is a full-time student.

10. Can I maintain coverage on an overage dependent child?

Yes. In order to maintain coverage on an overage dependent child, a specific form must be submitted to the insurance company for their approval. Please contact our office regarding your request at
1-800-216-1107.

11. What happens to my coverage if I am on a temporary leave of absence?

You can maintain your Enhanced life insurance coverage while on leave. As payroll deductions cease while on leave without pay, premiums must be provided to our office directly by personal cheque or pre-authorized payments.

12. Can I submit an application by email or fax?

No, our office will only accept an original ink signed PSAC Enhanced coverage application forms received by mail delivery.

13. How do I change my address?

Members can advise our office of their address change by email at [email protected] or by calling our toll free number at 1-800-216-1107.

14. Can I get details about the policy?

The details of the policy are located here. Members may also request their personal Certificate of Insurance by email at [email protected] or by calling our toll free number at 1-800-216-1107.

15. How much life insurance should I get?

As 3rd party administrators of the plan, we are not licensed to give financial advice so we recommend directing this question to a financial advisor.

16. Can I reduce or cancel my coverage?

Yes, you can reduce or cancel your coverage at any time by completing the "PSAC Optional Benefits Change Form". All changes to the plan are effective the 1st of the month following receipt of a signed and dated request.

17. What happens to my coverage when I retire?

As payroll deductions cease at retirement, premiums must be submitted to our office directly by personal cheque or pre-authorized payments in order to maintain the coverage. Please contact our office by email at [email protected] or by calling our toll free number at 1-800-216-1107 to make payment arrangements.

18. What happens to my coverage if I am no longer a member of PSAC?

If you are no longer a member of PSAC, premium payments must be submitted to our office directly by personal cheque or pre-authorized payments in order to maintain the coverage. Please contact our office by email at [email protected] or by calling our toll free number at 1-800-216-1107 to make payment arrangements.

19. Why did my premiums increase?

Premiums increase as the insured moves from one age category to the next. Please refer to the monthly rate table for details.

20. How much life insurance do I have?

Please contact our office by email at [email protected] or by calling our toll free number at 1-800-216-1107 to request your Certificate of Insurance. Your certificate will be sent to you by mail and will provide you with details regarding your plan.

21. How do I change my beneficiary?

Simply complete the "Change of Beneficiary and Name Form" and return the original ink signed form to our office by mail only at the following address:

22. Who can I name as beneficiary?

You may name whomever you choose. You may name a beneficiary or your Estate, but not both. If you wish name a charity, the charitable donation number is required in order to process your request. A Financial Institution cannot be named as a beneficiary.

23. What is an irrevocable beneficiary?

An irrevocable beneficiary designation cannot be changed unless the irrevocable beneficiary provides written consent to make a change.

An irrevocable beneficiary who is a minor child cannot provide written consent until he or she reaches the age of majority in his or her province of residence.

Where Quebec law applies and you have designated your married spouse or civil spouse as beneficiary, the designation will be irrevocable unless you select the designation as revocable.

24. How do I change my name?

Simply complete the "Change of Beneficiary and Name Form" and return it to our office along with a clear copy of valid identification confirming the name change by email to [email protected], by fax to 1-800-613-1234 or by mail to the following address:

25. How do I report a specified loss as a result of an accident?

We ask that the member contact our office at 1-800-216-1107.

26. How do I, or my loved one, report a death (Member, Spouse, Child)?

We ask that the beneficiary or Executor of the Estate contact our office at 1-800-216-1107.

27. What would happen to my spouse's coverage if I were to pass away?

The insured spouse has the option to convert the coverage into an individual life insurance policy with Manulife Financial with no medical evidence of insurability provided the 1st payment is remitted to the insurer within 31 days of the member's date of death.

28. What happens to my coverage if I become totally disabled and unable to work?

You may maintain your coverage and you may also be eligible for a waiver of premium benefit. Members, who are disabled for at least 9 consecutive months, but within 36 months, may apply for the waiver benefit. During this time, members are responsible for paying premiums directly to our office while awaiting approval of the waiver of premium benefit. Please contact our office at 1-800-216-1107 to advise us of your leave and to obtain additional information.

29. Will quitting smoking impact my premiums?

Yes, there are lower monthly premiums associated with a health status as a "non-smoker". When you have quit smoking for 12 consecutive months, the insured is required to complete and submit the "Non-Smoking Declaration" form to our office. The change in premium will only take effect the 1st of the month follow receipt of the completed form.

30. Can I keep the life insurance coverage for my ex-spouse now that we are divorced?

If you divorce from your insured spouse, the former spouse will remain insured under the plan until you cancel the coverage or until the coverage terminates on October 1st following the insured's 70th birthday.

31. When does my policy end?

Failure to provide regular monthly premium payments to our office will result in the termination of the policy. However, if regular monthly premium payments are provided to our office, the policy will end on October 1st following the insured's 70th birthday.

32. How can I convert my coverage after it ends?

The insured member has the option to convert the coverage into an individual life insurance policy with Manulife Financial with no medical evidence of insurability provided the 1st payment is remitted to the insurer within 31 days of the termination date.

33. How do I convert my coverage after it ends?

If you have any questions regarding conversion or wish to convert your coverage, one of our Individual Financial Services (IFS) Consultants will be glad to assist you. Simply call our IFS Coordinator at 1-888-613-1234 at extension 4991 and your call will be directed to an IFS Consultant.
