FAQ

1. What is my PSAC ID number?

Your PSAC ID number is located on your PSAC Membership Card. If you do not have your card on hand, you may contact our office at 1-800-216-1107 or contact your PSAC Regional Office.

2. How do I apply for the PSAC Free $10,000 life insurance coverage?

Simply complete the PSAC Free $10,000 Enrolment Card and return it to our office by mail only to the following address:

3. Can my spouse apply for the PSAC Free $10,000 life insurance coverage?

Only PSAC members can complete a PSAC Free $10,000 Enrolment Card.

4. Is the PSAC Free Basic coverage really free for members?

The PSAC Free Basic life insurance coverage is sponsored by the Public Service Alliance of Canada and is free for members of the PSAC.

5. Is this coverage automatic?

No, members must complete a PSAC Free $10,000 Enrolment Card in order to enrol in the coverage.

6. Can I submit my completed enrolment card by email or fax?

No, our office will only accept an original ink signed enrolment card received by mail delivery.

7. How do I change my address?

Members can advise our office of an address change by email at [email protected] or by calling our toll-free number at 1-800-216-1107.

8. Can I get details about the policy?

The details of the policy are located here. Members may also request their personal Certificate of Insurance by email at [email protected] or by calling our toll-free number at 1-800-216-1107.

9. Can I get more life insurance beyond the Free $10,000?

Yes, PSAC members in good standing who are actively at work and under the age of 65 are eligible to apply for the PSAC Enhanced Life Insurance from $25,000 to a maximum of $250,000.

10. What happens to my Free $10,000 life insurance coverage when I retire?

The PSAC Free $10,000 terminates on the day you retire.

11. What happens to my coverage if I am no longer a member of PSAC?

The PSAC Free $10,000 coverage ends with the termination of your employment and/or your membership of PSAC.

12. What heppens to my coverage if I am on a temporary leave of absence?

The PSAC Free $10,000 can be maintained for a temporary leave of absence only. Members can submit a "Membership in Good Standing Request" to the union directly at http://psacunion.ca/migs.

13. How do I change my beneficiary?

Simply complete the "Change of Beneficiary and Name Form" and return the original ink signed form to our office by mail only at the following address:

14. Who can I name as beneficiary?

You may name whomever you choose. You may name a beneficiary or your Estate, but not both. If you wish to name a charity, the charitable donation number is required in order to process your request. A Financial Institution cannot be named as a beneficiary.

15. How do I change my name?

Simply complete the "Change of Beneficiary and Name Form" and return it to our office along with a clear copy of a valid identification confirming the name change by email to [email protected], by fax to 1-800-613-1234 or by mail to the following address:

16. What is an irrevocable beneficiary?

An irrevocable beneficiary designation cannot be changed unless the irrevocable beneficiary provides written consent to make a change.

An irrevocable beneficiary who is a minor child cannot provide written consent until he or she reaches the age of majority in his or her own province of residence.

Where Quebec law applies and you have designated your married spouse or civil spouse as beneficiary, the designation will be irrevocable unless you select the designation as revocable.

17. How do I make a claim for a specified loss as a result of an accident?

We ask that the member contact our office at 1-800-216-1107.

18. If I die, how does my beneficiary/Executor make a claim?

A beneficiary or Executor is asked to contact our office at 1-800-216-1107.

19. When does this policy end?

This policy ends with the termination of your employment and/or membership in PSAC; your retirement; your death; the termination of the policy; when you cease to be an insurable member; the 1st of the month coincident with or immediately following the member's 70th birthday.

20. How can I convert my coverage after it ends?

The insured member has the option to convert the coverage into an individual life insurance policy with Manulife Financial with no medical evidence of insurability provided the 1st payment is remitted to the insurer within 31 days of the termination date.

21. How do I convert my coverage after it ends?

If you have any questions regarding conversion or wish to convert your coverage, one of our Individual Financial Services (IFS) Consultants will be glad to assist you. Simply call our IFS Coordinator at 1-800-613-1234 at extension 4991 and your call will be directed to an IFS Consultant.
